The APS6 Liaison Officer will assist staff working within health and justice settings to support prospective and existing participants transitioning back into the community.

Duties:
Liaise with Agency business areas to ensure participants have appropriate supports in place when transitioning into communities
Address challenges or delays in access or planning decisions, ensuring timely and safe transitions back into the community
Support potential participants in hospital or justice settings to access the Agency
Develop initiatives to build networks across the health or justice sector
Raise awareness and represent the Agency at networking and cross-agency forums
Provide clear and regular communications to external stakeholders in health or justice settings
